we have heard that some of the thrombosis seen in these patients may relate to dehydration but also from the fact that the virus can damage the endothelium directly as they have ACE2 expression and so people are hypercoaguable. Are we now rountinely anticoagulated people without contraindications?

There are prior studies on early tracheostomy within the first week can reduce ICU time and days on vent (Griffiths et al BMJ 2005, Rodriquez et al surgery 1990, Rumbak et al Critical Care Med 2004). Is something to consider for COVID with ARDS where we may expect a prolonged course of ventilator dependence?

Andrew - I don’t know about a protocol but I had a patient who was DNR/DNI last week who was going to get HFNC on the floor - all set up - but then made CMO. So I know it can happen.
