I think shame and embarrassment is a big issue in health care and especially with low health literacy. Patients don't want to appear "dumb" and shake their heads yes even if they don't understand.

I am unsure if I have ID or not. I tried to get diagnosed but as an adult I was only allowed a Psyc evaluation which has consistency of ID when I experience severe high anxiety
